Key takeaways

  • Your actual working arrangements determine the tax treatment; choosing a package does not change your employment status.
  • Keep records for each income source and check the official guidance linked to the questions below.
  • Agree the accounts, returns and advisory work you need before accepting a written fixed-fee quote.

Which accountancy support fits your income?

Start with how you are paid. Different income sources can need different accounts, returns and expense checks.

Sound familiar?

Problems we fix for allied health professionals and therapists

  • "I'm going self-employed for the first time."
  • "I have both NHS and private income."
  • "I don't know whether my services are VAT exempt."
  • "When should I go limited?"
  • "Can I claim my regulator fees?"
  • "Making Tax Digital starts from April 2026."

What it's costing you

Leaving it unfixed is rarely free

Overpaid tax
Unclaimed expenses and wrong tax codes mean paying more than you owe, year after year.
HMRC penalties and interest
Late returns attract automatic penalties, plus interest on tax paid late.
Lost evenings
Hours spent on forms, invoices and spreadsheets after a long shift.

General examples — your position depends on your own figures.

How we help

01

Mixed-income tax returns

Bring PAYE, agency and self-employed practice income together without double-counting tax already deducted.

02

Professional expenses

Review registration fees, eligible travel, equipment and other costs under the correct employment or self-employment rules.

03

Private-practice accounts

Simple records and accounts for room rental, patient fees and business costs.

04

Digital records

Check Making Tax Digital requirements and choose a manageable record-keeping workflow.

What to know before you decide

Agency work is not always self-employment

A nurse or allied health professional may be paid through PAYE by an agency or umbrella even while describing the work as freelance. Check payslips and contracts before registering a business or treating all receipts as untaxed income. If you also see patients privately, those receipts may need separate self-employment records and tax reporting.

Expense relief follows the rules for your role

The same travel or training cost can receive different treatment depending on employment status and its purpose. Professional registrations, equipment, uniforms and room rental should be reviewed, not claimed automatically. VAT exemption for private healthcare also depends on professional registration and the nature and purpose of the service; a professional title alone does not settle every supply.

Discuss this package

Employment status is determined by the facts, not the package you choose. PAYE, umbrella and partnership arrangements may also apply. A limited company does not override IR35 or off-payroll rules, and not every employed clinician needs a tax return.

Overpaid tax?

Tax refund support for nurses

We review tax codes, eligible professional expenses and overpaid tax, then prepare a claim where you qualify. A refund is not guaranteed; HMRC decides the claim.

£249 + VAT

For refunds up to £5,000, our fee is £249 plus VAT. For refunds above £5,000, add 5% of only the amount above £5,000 to the £249 fee. VAT applies to the total professional fee.

Example: a £6,000 refund means £249 + £50 = £299 plus VAT.

£249

Ask about a tax refund

This illustrates the fee only, not whether you qualify or how much HMRC will refund. We agree the claim scope and payment terms before work starts.

Your questions, answered

Do employed nurses always need a tax return?
No. PAYE-only employment does not automatically require Self Assessment. Untaxed income and other HMRC criteria need checking.
Can you help physios, podiatrists and paramedics?
Yes, with agreed tax and accounting work reflecting your profession, status and income sources.
Can I claim every training course?
No. The nature of the training and the employed or self-employed rules determine whether relief is available.
