What does an accountant for endodontists and periodontists do?

Key takeaways

  • Equipment, vehicles and fit-out costs often qualify for capital allowances, giving tax relief as the assets are bought.
  • For endodontists and periodontists, the choice between sole trader and limited company turns on profit level, how much income you draw, and whether your work is NHS employment, private practice or outside-IR35 locum work.

Which accountancy support fits your income?

Start with how you are paid. Different income sources can need different accounts, returns and expense checks.

  • "HMRC has opened an enquiry and I don't know what to send."

What it's costing you

Leaving it unfixed is rarely free

Overpaid tax
Unclaimed expenses and wrong tax codes mean paying more than you owe, year after year.
HMRC penalties and interest
Late returns attract automatic penalties, plus interest on tax paid late.
Lost evenings
Hours spent on forms, invoices and spreadsheets after a long shift.

General examples — your position depends on your own figures.

What to know before you decide

Referral income

This is one of the questions we work through with endodontists and periodontists at the start of every engagement. We look at your real income, contracts and records, set out the options in plain English and confirm the work and fee in writing.

Associate vs principal

This is one of the questions we work through with endodontists and periodontists at the start of every engagement. We look at your real income, contracts and records, set out the options in plain English and confirm the work and fee in writing.

Equipment

Equipment, vehicles and fit-out costs often qualify for capital allowances, giving tax relief as the assets are bought. The Annual Investment Allowance can give full relief in the year of purchase on qualifying spending. We track assets and claim the right allowance.

Incorporation

For endodontists and periodontists, the choice between sole trader and limited company turns on profit level, how much income you draw, and whether your work is NHS employment, private practice or outside-IR35 locum work. A company can defer tax on retained profit but adds accounts, payroll and filing costs. We compare both on your figures before you decide.

Your questions answered

How are dental practices valued?

Healthcare businesses such as pharmacies, dental practices and care homes are usually valued on a multiple of sustainable profit (EBITDA), adjusted for property, contracts, regulatory rating and goodwill. Clean, consistent accounts support a stronger price. We prepare normalised figures for sales or acquisitions.

Should I incorporate my dental practice?

A limited company can suit endodontists and periodontists with substantial private or outside-IR35 income that is not all needed to live on, because profit is taxed at corporation tax rates and can be paid out over time. It brings extra accounts, payroll and filing costs, and NHS employment income cannot move into it. We model sole trader against limited company on your real figures first.

How is goodwill taxed when buying or selling a practice?

Endodontists and periodontists pay income tax on taxable profit at 20%, 40% and 45%, plus Class 4 National Insurance on self-employed profit. Payments on account mean your first bill can include half of next year's tax too. Where income also comes from overseas, residence status and split year treatment decide what the UK taxes.

What due diligence is needed before buying a dental practice?

Financial due diligence involves reviewing the last three years of accounts and tax returns. You should also verify UDA performance and staff contracts. Legal due diligence covers the property lease and CQC registration status. Medical.Accountants assists buyers by analysing these financial records to ensure transparency during the acquisition process.

How can I fund a dental practice purchase?

Funding for a dental practice purchase can be secured through specialist healthcare lenders, often requiring a deposit. Lenders will review the practice's historical accounts and your own professional experience. Medical.Accountants prepares the necessary financial due diligence and business plans to support your application. We help you understand the tax implications of different financing structures.

How does VAT partial exemption work for practices with cosmetic income?

Most health care by registered professionals is VAT exempt where the main purpose is protecting or restoring health, so many clinicians never register. VAT becomes relevant when you sell goods, provide cosmetic or medicolegal services, or exceed the registration threshold with taxable income. HMRC Notice 701/57 gives the official guidance.

How should NHS contract income and UDAs be accounted for?

NHS contract income, including PCN, ARRS and UDA or orthodontic contract payments, must be accounted for in the period it is earned, with clawbacks provided for. Partners' profit shares and drawings are kept separate. Accurate accounts also drive the pension certificates each partner submits.

Does Business Asset Disposal Relief apply when I sell my practice?

Business Asset Disposal Relief may apply to the sale of a qualifying dental practice, potentially reducing the capital gains tax rate. Specific eligibility criteria must be met regarding ownership duration and business structure. We evaluate your situation as part of our fixed-fee compliance service. Proper documentation is essential for any claim.

What are the costs of setting up a squat practice?

Setting up a squat practice involves significant capital expenditure for premises, dental equipment, and regulatory compliance. Costs vary widely based on location and the scale of the fit-out. You must also budget for professional fees and CQC registration. Medical.Accountants provides remote UK-wide support to help track these start-up costs accurately.

How should associates and nurses be paid?

Associates are typically paid as self-employed individuals based on a percentage of their gross fees minus expenses. Nurses are usually employees and must be paid through a PAYE scheme. Employers may benefit from the £10,500 employment allowance to reduce National Insurance costs. We manage payroll services for healthcare businesses through our fixed-fee packages.

Discuss this package

Employment status is determined by the facts, not the package you choose. PAYE, umbrella and partnership arrangements may also apply. A limited company does not override IR35 or off-payroll rules, and not every employed clinician needs a tax return.

Overpaid tax?

Tax refund support for healthcare professionals

We review tax codes, eligible professional expenses and overpaid tax, then prepare a claim where you qualify. A refund is not guaranteed; HMRC decides the claim.

£249 + VAT

For refunds up to £5,000, our fee is £249 plus VAT. For refunds above £5,000, add 5% of only the amount above £5,000 to the £249 fee. VAT applies to the total professional fee.

Example: a £6,000 refund means £249 + £50 = £299 plus VAT.

Your questions, answered

What does an accountant for endodontists and periodontists do?
An accountant for endodontists and periodontists prepares your accounts and tax returns, keeps you compliant with HMRC deadlines and advises on the decisions specific to your work: how income is structured, which expenses are allowable, VAT where it applies and, for NHS scheme members, pension tax. At Medical.Accountants the work is quoted as a fixed monthly fee, agreed in writing before we start.
What can endodontists and periodontists claim against tax?
You can claim tax relief on costs incurred wholly and exclusively for your work: HMRC-approved professional fees such as GMC, NMC, GDC or HCPC registration and approved royal college and union subscriptions, uniform laundering, and business mileage between workplaces. Ordinary commuting and most exam or training costs for new qualifications are not allowable. Employees can claim back up to four tax years.
Do endodontists and periodontists need an accountant?
Many endodontists and periodontists must file a Self Assessment return: you need one if you have untaxed income such as private, locum or self-employed work. A high PAYE salary alone does not require Self Assessment for 2026/27; check HMRC's current criteria and any notice to file. The online filing and payment deadline is 31 January after the tax year ends. Under the non-MTD rules, late filing normally triggers an initial £100 penalty, with further penalties for longer delays. A different regime applies from the tax year you join MTD for Income Tax.
How much does an accountant for endodontists and periodontists cost?
Our packages for endodontists and periodontists start at £45.83 + VAT a month, with Core at £62.50 and Growth at £75 + VAT. The right package depends on your income sources, whether you trade through a company and any payroll, VAT or pension work. We confirm the exact fee in writing before any work begins, so there are no surprise invoices.
Do endodontists and periodontists need to comply with Making Tax Digital?
Making Tax Digital for Income Tax applies from 6 April 2026 to sole traders and landlords whose qualifying income (gross self-employment plus property income) is over £50,000, falling to £30,000 from April 2027 and £20,000 from April 2028. PAYE salary does not count. Endodontists and periodontists trading through a limited company or a partnership are not brought in yet. We set up compatible software and file the quarterly updates for you.
