Problems we fix for locum doctors

  • "I don't know if I should be a sole trader, a limited company or use an umbrella."
  • "A trust or agency has sent me an IR35 determination I don't understand."
  • "My first tax bill was huge because of payments on account."
  • "I keep missing or getting my locum A/B and Type 2 pension forms wrong."

  • "I'm not sure what I can claim — indemnity, GMC, mileage?"
  • "Making Tax Digital quarterly updates start from April 2026 and I'm not ready."

What it's costing you

Leaving it unfixed is rarely free

Overpaid tax
Unclaimed expenses and wrong tax codes mean paying more than you owe, year after year.
HMRC penalties and interest
Late returns attract automatic penalties, plus interest on tax paid late.
Lost evenings
Hours spent on forms, invoices and spreadsheets after a long shift.

General examples — your position depends on your own figures.

Payroll & Pensions for Doctors & Healthcare Professionals

Pensionable earnings

For 2026/27 the standard annual allowance is £60,000. It tapers where threshold income exceeds £200,000 and adjusted income exceeds £260,000, by £1 for every £2, to a minimum of £10,000. NHS pension growth is measured as a defined benefit input, so a pay rise or promotion can trigger a charge without any extra contributions. We run the calculation before the year ends and explain Scheme Pays where a charge arises.

Expenses and mileage

Allowable costs include HMRC-approved professional registrations and subscriptions, indemnity where you pay it, business mileage between workplaces and equipment used for work. Ordinary commuting and personal costs are not allowable. Employees claim through HMRC; the self-employed deduct costs in their accounts.

Your questions answered

What are GP locum A and B forms and when are they due?

GP partners report NHS pensionable profit to the scheme each year on a certificate based on the practice accounts; locum GPs use the locum A and B forms to pay contributions on each piece of pensionable work. Late or wrong forms cause missing service and incorrect pension records. We prepare the figures and check them against the accounts before submission.

How are pensionable earnings calculated for locum GPs?

Most NHS doctors and many healthcare staff are members of the NHS Pension Scheme. Contributions are deducted through payroll, and locum or private work can count only through the correct forms. High earners should check the annual allowance each year, as pension growth can trigger a tax charge.

Should a locum doctor be a sole trader or a limited company?

A limited company can suit locum GPs with substantial private or outside-IR35 income that is not all needed to live on, because profit is taxed at corporation tax rates and can be paid out over time. It brings extra accounts, payroll and filing costs, and NHS employment income cannot move into it. We model sole trader against limited company on your real figures first.

Is locum work through an agency or trust bank inside IR35?

For locum engagements with NHS bodies, the hirer must decide IR35 status and issue a Status Determination Statement; the small-company exemption does not apply to public bodies. The test looks at control, substitution and mutuality of obligation in the actual working arrangement. You can ask the hirer to review a determination you disagree with, and it must respond within 45 days.

How do I register as self-employed with HMRC when I start locum work?

Employment status depends on the real working relationship: who controls the work, whether you can send a substitute, whether you carry financial risk and how integrated you are. A contract label alone does not decide it. HMRC's Check Employment Status for Tax tool and case law set the tests, and getting it wrong can leave the engager owing PAYE.

What expenses can locum doctors claim against tax?

You can claim tax relief on costs incurred wholly and exclusively for your work: HMRC-approved professional fees such as GMC, NMC, GDC or HCPC registration and approved royal college and union subscriptions, uniform laundering, and business mileage between workplaces. Ordinary commuting and most exam or training costs for new qualifications are not allowable. Employees can claim back up to four tax years.

How much of my locum income should I set aside for tax?

You should set aside a portion of every invoice to cover Income Tax, National Insurance, and potential student loan repayments. The exact percentage depends on your total annual earnings and tax bracket. Remember that payments on account are due every 31 January and 31 July. Regular monitoring ensures you have sufficient funds for these deadlines.

Why is my first tax bill as a locum so high (payments on account)?

Your first bill often includes the tax for the previous year plus the first payment on account for the following year. Payments on account are due on 31 January and 31 July. Each payment is normally half of the previous year's total tax liability. This can effectively result in paying 150% of your tax in one go.

What are payments on account?

Payments on account are advance payments towards your next Self Assessment bill, normally due on 31 January and 31 July. Each is usually half the previous year’s relevant Income Tax and Class 4 National Insurance bill, excluding Capital Gains Tax and student loans. Exceptions apply where the bill is below £1,000 or over 80% was collected at source.

Employment status is determined by the facts, not the package you choose. PAYE, umbrella and partnership arrangements may also apply. A limited company does not override IR35 or off-payroll rules, and not every employed clinician needs a tax return.

Tax refund support for locum GPs

We review tax codes, eligible professional expenses and overpaid tax, then prepare a claim where you qualify. A refund is not guaranteed; HMRC decides the claim.

£249 + VAT

For refunds up to £5,000, our fee is £249 plus VAT. For refunds above £5,000, add 5% of only the amount above £5,000 to the £249 fee. VAT applies to the total professional fee.

Example: a £6,000 refund means £249 + £50 = £299 plus VAT.
