Accountants for consultants in private practice

Most consultants combine NHS work with private patients, so salary, private profits and the NHS pension all affect one another. We plan them together — structure, pension taper and the £100,000 personal allowance trap — for a fixed fee.

Should a consultant run private practice through a limited company?

It can reduce tax at higher private profits and lets you time income, but it adds running costs and changes how pension and dividends work. We compare sole trader and company figures for your actual income first.

Fixed fees, specialist healthcare accountants, and every HMRC and NHS Pensions deadline handled.

  • Fixed monthly fee — no surprises
  • Specialist in NHS pension and healthcare tax
  • Onboarding within 48 hours

Medical.Accountants by ACCOTAX, ICAEW/ACCA-regulated chartered accountants, London, serving clients across the UK. Over 5,000 clients served.

Key takeaways

  • Private profits and NHS pay together decide your NHS pension annual allowance.
  • Income over £100,000 loses £1 of personal allowance for every £2 above.
  • Private practice income needs a Self Assessment return each year.

Sound familiar?

Problems we fix for consultants and specialists

  • "The tapered annual allowance and Scheme Pays confuse me."
  • "Should my private practice be sole trader, limited or an LLP?"
  • "I'm losing my personal allowance above £100,000."
  • "I'm unsure whether my medicolegal work needs VAT."
  • "Insurer and hospital billing never reconciles."
  • "I don't know the most efficient way to take profit out."

What it's costing you

Leaving it unfixed is rarely free

Overpaid tax
Unclaimed expenses and wrong tax codes mean paying more than you owe, year after year.
HMRC penalties and interest
Late returns attract automatic penalties, plus interest on tax paid late.
Lost evenings
Hours spent on forms, invoices and spreadsheets after a long shift.

General examples — your position depends on your own figures.

How we solve it

What we actually do for you

Pension modelling

Annual allowance and taper calculations before the year ends, not after.

Private practice set-up

The right structure for your private income, explained in plain English.

VAT review

We separate exempt medical care from standard-rated medicolegal work.

Income planning

Salary, dividends and pension contributions planned together.

How we help

01

Structure comparison

Sole trader against limited company, worked out on your NHS salary and private profits.

02

NHS pension taper checks

Threshold and adjusted income calculated every year, before the Pension Savings Statement arrives.

03

Self Assessment and accounts

Private practice accounts and your tax return prepared and filed, with payments on account planned.

04

Medical secretary and payroll

Payroll for secretaries and staff, run monthly with workplace pension.

What to know before you decide

The £100,000 trap

Between £100,000 and £125,140 of adjusted net income, the loss of personal allowance gives an effective 60% rate. Pension contributions and Gift Aid reduce adjusted net income and can restore it.

Tax Planning & Reviews for Doctors & Healthcare Professionals

Pension annual allowance

The standard allowance is £60,000 for 2026/27, tapering above £260,000 adjusted income to a minimum of £10,000. Unused allowance from the three previous years can usually be carried forward.

Payments on account

Private income usually brings payments on account: half on 31 January and half on 31 July, based on last year's bill.

Your questions answered

Are my services exempt or standard-rated?

Medical care provided by registered health professionals is generally VAT exempt if the primary purpose is protecting health. Purely cosmetic procedures and medicolegal reports are typically standard-rated at the prevailing rate. You must monitor your taxable turnover against the £90,000 registration threshold. Determining the correct liability requires a detailed review of your specific services.

What is partial exemption?

Partial exemption applies when a healthcare business makes both VAT-exempt and taxable supplies. You cannot usually reclaim all input VAT if you have exempt income. A specific calculation determines how much VAT on overheads is recoverable. This is a common requirement for GPs and consultants with mixed income streams.

What happens if I registered late?

Failure to register for VAT when your taxable turnover exceeds £90,000 may result in financial penalties. HMRC may also charge interest on the VAT that should have been paid from the effective date of registration. You will be liable for the VAT on past sales regardless of whether you charged it. Early disclosure often helps in mitigating these penalties.

Discuss this package

Employment status is determined by the facts, not the package you choose. PAYE, umbrella and partnership arrangements may also apply. A limited company does not override IR35 or off-payroll rules, and not every employed clinician needs a tax return.

Overpaid tax?

Tax refund support for medical consultants

We review tax codes, eligible professional expenses and overpaid tax, then prepare a claim where you qualify. A refund is not guaranteed; HMRC decides the claim.

£249 + VAT

For refunds up to £5,000, our fee is £249 plus VAT. For refunds above £5,000, add 5% of only the amount above £5,000 to the £249 fee. VAT applies to the total professional fee.

Example: a £6,000 refund means £249 + £50 = £299 plus VAT.

This illustrates the fee only, not whether you qualify or how much HMRC will refund. We agree the claim scope and payment terms before work starts.

Your questions, answered

Do I need an accountant if I have only a little private work?
Not necessarily, but even small private income can push you over the £100,000 or pension taper limits. A short review shows whether it does.
Can my spouse help in the practice?
Yes, if they do real work for fair pay. We set it up properly through payroll.
